How and when Yuan Xi is born is not detailed in the "once-upon-a-time" story Zhenie (Ai-Ai delas Alas) is narrating to Jade (Mutya Orquia).
The pilot episode of My Binondo Girl immediately answered my question. I knew the "one-upon-a-time" story is just made up. Yuan is Zhenie's second born, who died on his birth. And he's the reason why Jade wants to be a boy.

Here is the real story that Zhenie withheld from Jade:
Zhenie is an english teacher in China. There she met Chen Sy (Richard Yap), who at that time is not yet rich and only sells siopao and noodles. Zhenie taught Chen Sy english in exchange for free siopao and noodles. But they also fell in love with each other.
When Zhenie got pregnant, Chen Sy brought her to his family. They were reluctant to accept her but when Chen told them Zhenie was pregnant with a boy, they got excited. However, Zhenie gave birth to a girl she named Jade. They believed a first born girl is bad luck.
In a taxi on her way home with her mother Amor (Gina Pareno), Zhenie felt her womb hurt and her mother brought her to the hospital. The boy, they named Yuan because it is his father's name for him, died at birth.
After Jade's fifth birthday, Chen came back to the Philippines, now very rich, looking for Yuan. This is how Jade knew the real story. And this is why she wanted to became a boy. She wanted to be with her father. Tweet
